Enfermera Homecare (Programa de Soporte a Pacientes)

Actualmente nos encontramos en la búsqueda de profesionales del área de la salud que quieran ser parte de un gran proyecto con una importante farmacéutica multinacional.

Objetivo del cargo

Responsable de garantizar el cumplimiento de la atención y adherencia del paciente al programa.

Principales Funciones

  • Recolectar consentimiento firmado en físico, una vez el paciente nuevo sea contactado telefónicamente.
  • Educar al paciente de acuerdo a las necesidades identificadas, en Acceso /Educación en Salud/ Tratamiento / Adherencia Terapéutica
  • Educar a los pacientes sobre la aplicación de medicamentos por vía subcutánea
  • Realizar visita o llamada semanal a pacientes en estado de suspendido y acceso para educar, orientar, motivar y generar las actividades para lograr el acceso al medicamento.
  • Registrar en el CRM los cambios de estado y anotar de forma clara y concisa los comentarios del seguimiento del paciente donde se evidencie estado y necesidades identificadas.
  • Generar alarmas por barreras de acceso de los pacientes en tiempo real.
  • Generar reporte de Eventos Adversos y/o Problemas de Calidad
  • Visitar las IPSS y Médicos de acuerdo al plan establecido.
  • Entregar el material educativo y Kits de inicio a los pacientes.                                                      

Requisitos

  • Carrera técnica o tecnológica en Enfermería
  • Experiencia mínima de un año en actividades relacionadas al tratamiento y adherencia del paciente a programas de soporte en el Sector Salud.
  • Compromiso con la educación del paciente
  • Sólidas habilidades de comunicación efectiva, orientación al cliente, planificación y trabajo en equipo
  • Ciudad base Medellín
