Aux. Enfermería Gestión Remota

Objetivo del cargo

Comunicación remota para seguimiento y educación a pacientes y cuidadores en procesos relacionados a trámites ante asegurador, corresponsabilidad, autocuidado, entendimiento de la enfermedad y manejo de medicamento; orientado a favorecer la adherencia de los pacientes a su tratamiento médico y control de la enfermedad.

Programación, coordinación y verificación de servicios prestados al paciente desde el PSP, de acuerdo con los procesos establecidos en el mismo.

Garantizar una comunicación asertiva y ética que aporte al cumplimiento del equipo y permita a los pacientes identificar en el PSP un apoyo a su proceso de salud enfermedad.

Requisitos

  • Carrera técnica o tecnológica en Enfermería
  • Experiencia mínima de un año en actividades relacionadas al tratamiento y adherencia del paciente a programas de soporte en el Sector Salud.
  • Compromiso con la educación del paciente
  • Sólidas habilidades de comunicación efectiva, orientación al cliente, planificación y trabajo en equipo
  • Conocimiento en los procesos actuales del sistema de salud y seguridad social en el país para la formulación de medicamentos de alto costo.
  • Ciudad base Bogotá.

Principales Funciones

  • Perfilación del paciente al momento de ingresar al PSP
  • Contacto telefónico al 100% de los pacientes, mínimo dos veces al mes
  • Identificar barreras de los pacientes que ocasionan suspensión del tratamiento
  • Generar consentimiento informado telefónico para pacientes nuevos
  • Identificación oportuna de eventos adversos o situaciones relacionadas que afecten la salud del paciente, realizando el respectivo reporte de acuerdo con proceso establecido.
  • Garantizar la confiabilidad y actualización de la información registrada en la herramienta dispuesta.
  • Programar trabajo de campo o contacto telefónico de enfermeras basado en necesidades de los pacientes de acuerdo con su estatus de tratamiento.
  • Activar vouchers de pruebas de laboratorio
  • Generar alertas por barreras de acceso de los pacientes en tiempo real
  • Generar reporte de Eventos Adversos y/o Problemas de Calidad
  • Generar otro tipo de soporte y educación requerido por los pacientes o el equipo de campo
